Personal Health Record (PHR)

The InteliChart Personal Health Record (PHR) is a secure online portal that allows patients to manage their personal health information in a reliable, centralized location.  Unlike other stand-alone PHRs, the InteliChart Personal Health Record is fully integrated and powered by the InteliChart EMPI and the InteliChart Integration Engine.

After patients are positively identified, they can conveniently update their current data by authorizing a search for all regional healthcare providers and organizations that they have visited.  When the InteliChart EMPI registers a patient match, an authorization request is forwarded to the identified organizations and if they are also licensed InteliChart subscribers, they will update the patient’s PHR with information from their own EHR databases. The InteliChart PHR will continue to be updated as providers amend their patients’ EHR records.
